ARC After School Program - Policies & Guidelines Enrollment Form General Information Arc After School Care Program provides supervision for school aged children with disabilities who are too old for services provided by conventional day care centers (age 13 - 21). The project offers a safe environment for relaxation, recreation and constructive social interaction among participants after their school day. Your child will have access to exercise activities, videos (g-rated only), board games, arts & crafts, music, and recreation in the enclosed basketball court. Snacks will be provided each day. After school care will be provided on regular school days, Monday thru Friday, from 3:00 p.m. until 5:30 p.m. We do not provide service on school holidays, teacher workdays, or weekends. We do not accept children for drop in care. Tuition is $50 per week paid weekly, in advance, regardless of holidays except for school breaks lasting longer than three consecutive days ( i.e. spring break). Parent/guardian (responsible party) is required to arrange transportation for the student to and from the program site (655 S. 8th Street - Spindletop MHMR north campus - Recreation Room) Children will only be released to persons listed on the enrollment form. If anyone other than the parent/guardian (responsible party) or someone listed on the enrollment form is to pick up a child(ren), we need to be notified in writing or by a telephone call in advance. The person picking up the child(ren) may need to show a driver's license or other picture ID. If a court order prohibits a parent from picking up a child, we need a copy of the order on file. To protect confidentiality, the Arc of Greater Beaumont will not share information about a child or a child's family with anyone who is not authorized to receive this information. The Arc of Greater Beaumont carries Board of Directors and general liability insurance for its operations. Ability Requirement and Behavior Guidelines This program is designed for youth with mild to moderate mental retardation or developmental disability. We will not provide one-on-one supervision, medical care or therapeutic services. Children must be able to communicate his/her own needs and perform his/her own self care skills (feeding, toileting). Participants are expected to be emotionally and physically well and demonstrate behavior within acceptable guidelines. To assure a safe, fun opportunity for relaxation, recreation and constructive social interaction, the following behaviors will not be tolerated and may lead to discharge of a child from the program. Biting, scratching, kicking, fighting Wandering or running away Willful destruction of property Self-injurious behavior Tantrums, foul language No smoking allowed Enrollment and Discharge of Enrolled Children An enrollment application shall be completed and submitted with a $30 non-refundable enrollment fee. A child is eligible for the program if they meet the ability requirement and can comply with behavior guidelines. A child may be discharged from the Arc After School Care program for
reasons such as, but not limited to: The Arc will provide a one-week notice of intent to discharge a child from the program, except when the discharge results from failure to pay fees when due. Should parent/guardian (responsible party) remove the child during a notice period we initiate, fees will not be charged for the remaining unused days. Parent/guardian (responsible party) must give a one-week notice of their intent to withdraw a child, and will be required to pay for that week whether or not the child continues to attend. All outstanding fees must be paid. Payments and Refunds A $30 non-refundable enrollment fee is required and is due when the application is submitted. Checks should be made payable to the Arc of Greater Beaumont. Tuition is $50 per week paid weekly, in advance, regardless of school holidays except for two extended school breaks: Christmas and Spring Break. Tuition is paid in advance on Friday (or final day of week's service) for the following week's services. If there will be a third party payment, a special payment schedule will be arranged and detailed in a contract with the payer. Parent/guardian (responsible party) will be accountable for any specified co-payments or amounts unpaid by a third party payer. Parent/guardian (responsible party) may pay in advance beyond one week if they choose. A $20 return check fee will be charged for any checks returned. An extra fee is assessed for late pick up of a child, pro-rated at $10 per 30 minute delay and is due at the end of the current week. There are no reduced fees for multiple children from the same family. No refunds will be given for days when children do not attend due to illness or other reasons. Child Absence If your child will be absent, please call the Arc office at 784-5556 and leave a message informing us of the absence on the office machine. If a child who is scheduled to arrive at the center does not arrive within 45 minutes of the time expected and we have not been notified of an absence, we will attempt to contact the parent/guardian (responsible party) to determine the child's whereabouts. We will not provide transportation to or from the Arc after school program and children will not be allowed to leave the center unsupervised. Child Illness/InjuriesChildren who are ill are not to be brought to the Arc After School Care Program. If a child arrives ill or becomes ill at the center, the parent/guardian (responsible party) will be contacted immediately and asked to pick up their child at that time. Children may return to the center when they are symptom free. A staff person with CPR/first aid training is available at all times. Superficial injuries will be washed with soap and water and covered with a bandage or treated with ice. The minor injury will be reported to the authorized person who picks up the child that day. In the event of serious illness or injury, the Arc will immediately contact 911 for medical assistance. The parent/guardian (responsible party) will be contacted as soon as possible after contacting 911. Should an ambulance be needed, parents will be responsible for any costs. Medications will not be administered by Arc staff. Emergency Procedures Fires drills will be conducted quarterly. In case of an emergency that would require an evacuation, children will be evacuated through the nearest safe exit. Emergency contact information will be taken along by Arc staff and parents will be contacted as soon as possible. Application An enrollment form must be submitted with the $30 non-refundable enrollment fee. Only complete forms will be considered.
